| Long-term disabled parking permits are available to any individual with permanent mobility problems or certified legally blind. The disability must be certified by a
licensed chiropractor, physician, osteopathic physician, podiatrist or the Adjudication Office of the Veterans Administration. Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. The applicant must have
a valid Florida Driver License or Florida Identification Card. Permits are valid for four years. |

| Applicants must submit a completed application. There is no fee for issuance of a long-term disabled parking permit. One additional permit may be issued to persons who certify they are frequent travelers or are
certified as quadriplegic. |

Renewal Information for Long-term Disabled Parking Permits |

| Recertification of the individual's disability is required to renew a permit for an additional four years. Prior to expiration, a renewal notice will be mailed from the Florida Division of Motor Vehicles for long-term Disabled Parking Permits.
Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. One additional permit may be issued to persons who certify they are frequent travelers or are certified as quadriplegic. There is no fee for renewal of a long-term disabled parking permit. |

Temporary Disabled Parking |

| Temporary disabled parking permits are available to any individual with a temporary disability certified by a licensed chiropractor, physician, osteopathic
physician, podiatrist or the Adjudication Office of the Veterans Administration. Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. The applicant must have a valid Florida Driver License
or Florida Identification Card. Permits can be valid for up to six months, as determined by the signing physician. |
