FAQS for First Time Driver License and Identification Card Changes Effective January 1, 2010

To Transfer a Valid Out of State License to Florida: |
|
| The following is required to transfer a valid out of state license to a Florida Driver License: |
|
| 1. United States Birth Certificate (original or certified copy); OR valid United States Passport. |
| 2. Surrender of the out of state license. |
| 3. Social Security Card, or one of the following documents showing your social security number: W-2 form, pay check stub or Form 1099. |
| 4. Vision Test. |
| 5. Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement,lease agreement, |
| voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy. |

| 6. If the documentation submitted reflects any name change, an original or certified copy of your Marriage License, Divorce Decree or Court |
| Order,must be submitted. In some instances, more than one marriage license, divorce decree or court order document may be required |
| to show the chain of name changes. Photocopies cannot be accepted. |

| If the out-of-state license is expired, suspended or revoked, a written and/or road test may be required. |

To Apply For a First-Time Florida Identification Card: |

| The following is required to apply for a Florida Identification Card: |
|
| 1. United States Birth Certificate (original or certified copy); or valid United States Passport. |
| 2. Social Security Card, or one of the following documents showing your social security number: W-2 form, pay check stub or Form 1099. |
| 3. Applicant must be 5 years of age or older. If the purpose of the Florida Identification Card is to apply for a Disabled Parking Permit, |
| the age limit is waived. A certification of the disability must be provided. |
| 4. Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement,lease agreement, |
| voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy. |

the applicant has never been licensed by any state or county: |

| The following is required to apply for a Learner Permit: |

| For applicants 17 years or younger: |
|
| 1. United States Birth Certificate (original or certified copy) OR valid United States Passport. |
| 2. Social Security Card or one of the following documents showing your social security number: W-2 form, pay check stub, or Form 1099. |
| 3. Proof of completion of a traffic law substance abuse education course. |
| 4. Parental Consent Form either signed and notarized in advance, or signed in the presence of our examiner. Person signing the |
| consent form must be legal guardian. If other than parent, such as grandparent, proof of guardianship must be provided. |
| 5. Vision Test. |
| 6. 20-Question Road Rules Test and 20-Question Road Sign Test. Tests must begin no later than one hour before office closing time. |
| 7. Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement,lease agreement, |
| voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy. |
