TO APPLY FOR A FIRST-TIME FLORIDA DRIVER LICENSE
OR IDENTIFICATION CARD
NOTE: Effective September 1, 2009, most motor vehicle and driver license fees increased significantly. These fee increases were passed by the Florida Legislature and approved by the Governor to help balance the state budget. None of the additional money will stay with Pasco County Government or the Tax Collector's Office. FAQS for Driver License Changes Effective October 1, 2008
To Transfer a Valid Out of State License to Florida:
    The following is required to transfer a valid out of state license to a Florida Driver License:
       1.  United States Birth Certificate (original or certified copy); or valid United States Passport.
       2.  Surrender of the out of state license.
       3.  Social Security Card, or any of the following documents showing your social security number:
           Tax return, W-2 form, property tax statement, pay check stub, DD-214, or school record.
       4.  Vision Test.
       5.  Proof of residential address is required. Provide any of the following reflecting your name and address: deed,
           mortgage statement, lease agreement, voter registration, vehicle registration, W2 form, 1099 form or a
           homeowners, automobile, or life insurance policy. For a complete list of items,click Proof of Residential Address.
    If the out-of-state license is expired, suspended or revoked, a written and/or road test may be required.
    Other supporting documentation will be required if documentation submitted for
    identification reflects any name change.
To Apply For a First-Time Florida Identification Card:
    The following is required to apply for a Florida Identification Card:
       1.  United States Birth Certificate (original or certified copy); or valid United States Passport.
       2.  Social Security Card, or any of the following documents showing your social security number:
            Tax return, W-2 form, property tax statement, pay check stub, DD-214, or school record.
       3.  Applicant must be 5 years of age or older. If the purpose of the Florida
            Identification Card is to apply for a Disabled Parking Permit, the age limit
            is waived. A certification of the disability must be provided.
       4.  Proof of residential address is required. Provide any of the following reflecting your
            name and address: deed, mortgage statement, lease agreement, voter registration,
            vehicle registration, W2 form, 1099 form or a homeowners, automobile, or life insurance

To Apply for a First-Time Florida Driver License (also known as a Learner Permit) and
the applicant has never been licensed by any state or county:
    The following is required to apply for a Learner Permit:
    For applicants 17 years or younger:
       1.  United States Birth Certificate (original or certified copy); or valid United States Passport.
       2.  Social Security Card, or any of the following documents showing your social security number:
            Tax return, W-2 form, property tax statement, pay check stub, DD-214, or school record.
       3.  Proof of completion of a traffic law substance abuse education course.
       4.  Parental Consent Form either signed and notarized in advance, or signed
            in the presence of our examiner. Person signing the consent form must be legal guardian.
            If other than parent, such as grandparent, proof of guardianship must be provided.
       5.  Vision Test.
       6.  20-Question Road Rules Test and 20-Question Road Sign Test. Tests must begin
            no later than one hour before office closing time.
       7.  Proof of residential address is required. Provide any of the following reflecting your
            name and address: deed, mortgage statement, lease agreement, voter registration,
